Detail regarding 2nd mate

Pls give me detail as I have to go for 2nd mate,I have more than 40months of sea service but all in offshore vessel and also guide me the step for it. What is form 15 and form 3a is it necessary and from where I can get it

  offshore vessel seatime is not counted in full in india, better you go to uk , where its fully counted
