  Attached below the DCE Form (Page 2) with list of documents required. A thing worth mentioning is that you need your company letter for applying so better contact your company (with ship details and cargoes carried) so that they can mail you the same.   Dtz d procedure for renewal or getting new Dc but I have lost my operational level Dc ad now I want management level, so plz advise m d procedure
