MUI India Medical Claim

I am a member of both Maritime Union of India (MUI) and NUSI. I have been on medical treatment for last 4 months at AIIMS, Delhi. I need to claim my medical expense from MUI.
Can somebody tell how much is the maximum limit we can claim from Maritime Union of India ?
What is the procedure for Medical claim from MUI ?

  Open http://maritimeunionofindia.com/mnows.html  and download following forms

  1. Registration Form
  2. Declaration Form
  3. Domiciliary Treatment Form
  4. Hospitalisation Treatment Form
  5. Day care treatment form
  Fill above forms and send them to address mentioned in the form along with original medical fee receipts / reports / prescription.
  
  I have claimed for my wife's hospitalization from Maritime Union of India (MUI) few months back but came to know MUI will take more than 6 months to reimburse the same.
