CDC Renewal Form
Download the CDC Renewal form @ M.S. Notice 24 of 2012 Dated 04.12.2012 . To me CDC renewal form is self explanatory. It covers not only CDC Renewal but also duplicate CDC and lost CDC replacement procedure. Listing down important points
- No need of attestation by Gazetted Officer, Self attestation accepted on copies of your documents.
- Do not forget to write your Full name and date of self attestation.
- Use only CAPITAL LETTERS while filling your CDC Renewal form.
- Avoid over-writing !
- Fill & send only page number 4 to 8 (first 3 instruction pages are for self reference)
- Write on top left hand of the envelope (containing documents/CDC renewal form) in BLOCK LETTERS Replacement/Duplicate/Revalidation of CDC as required.
- No fees levied for re-validation of CDC (CDC Sticker).
- Write your name on the reverse of your photograph.
- Send the application by Registered / Speed post / Courier service to the Shipping Master (
from where CDC was obtained i.e. Mumbai/Kolkata/Chennai,address given in the CDC Renewal form or try DG Shipping Contact list.)

I copied all the filled and printed pages of CDC.
All documents list send by me attached below.
Kept the documents list simple, did not add covering letter or next of kin details (advised by few shipping companies)

1 What is 9. Copy of profile registration?

1 Copy of Profile registration is the complete printout of your e-samudra DG Shipping Seafarer Profile.

1 It is mentioned on the CDC Renewal form that passport size photograph is to be affixed on the application.... but it also says 3 nos. of 2.5x3cm stamp size photos are to be enclosed, one of which is to be affixed on the application what is the exact thing to be done.... pls reply urgently

3 stamp size photographs in white shirt (size 3 cm x 2.5 cm) - one should be affixed on the application form.
This confused me too. I did the following- Affixed 1 passport size photo (3.5 cm X 3.5 cm)
- Put 3 stamp size photo (3 cm X 2.5 cm) in envelope
- Wrote name on reverse of 3 stamp size photograph

0 also kindly advice, can I apply for renewal OF cdc even-though the period to expire the validity is almost 9 months... or in other words, I want to know should we apply for renewal after the CDC validity is expires or before to that... if before then what is the early period ( 1 year/ six months )

0 I lost all circular reference since DG Shipping website changed hence cannot link it. Logically GP rating contracts are for 9 months so CDC renewal should be allowed 10 months before. I also found mariners who applied almost an year before the CDC expiry and got the sticker.

Pls advice is it necessary to attest DG profile pages which was printed.
And also post card need to be Rs-5/ stamp instead of Rs-3/. Thanks - X
1 No harm in self attesting, that is what I did ! Trust post card stamp details given in form, though it does not matter because anyways if it is of lesser value, you only will be paying for it (it is returned back to you) !

Please advice if the self attested xerox copies of CDC to be enclosed is only the first two pages or the copies of all pages of sea-time record???? - X

DG website and guidelines doesnt mention about STCW course xerox copies and it had mentioned about CDC copy with gazetted officer attestation and DG profile. so is it a trend that shipping master rejects application without copy of modular certificates? - X
0 Hi Friend, most of the things I mentioned are already in the CDC Renewal Form. I am not sure what you are talking about or can you please leave the link of the concerned Guidelines which you are mentioning ! Shipping masters do not reject anything without a cause, and when they do, they do send cause of rejection letter which I have mentioned in CDC Renewal Status.
